Medical Records

We offer many ways for you to share your historical records from other providers.  You can upload your own records to the patient portal once you establish as a patient.  You or your former providers can fax records to either of the following numbers:

Electronic Fax: ‭(888) 876-4773‬

Athena Direct Fax: ‭(866) 777-2232‬

Lab Results: ‭(920) 489-2324‬

Many patients have asked, “What happened to Women’s Specialty Care and my medical records?” Wisconsin requires records to be kept for at least 5 years. All electronic records from 2009 at Women’s Specialty Care were maintained electronically in the Athena Health system. Between 2017 and 2021 the providers from Women’s Specialty Care utilized Bellin’s electronic system, Epic. In 2015, Dr. Coussons travelled to Washington DC to advocate for open sharing and intra-operability of your records when many electronic records were closed proprietary systems. In March 2018 Athena and Epic announced open intra-operability.  Athena connects to many outside records and data sources allowing us to seamlessly review most records.

In the fall of 2025 Advanced Care for Women will transition to a fertility specific EMR to support the complexity of IVF and the highly involved lab process to ensure the best and safest outcomes.   We will have access to your detailed medical records and billing history in Athena (2009-2017 and 2022-2025), but will sunset use of the Athena over the coming months.  Before completely turning off this EMR, we will archive all of the records electronically and keep them for at least 7 more years.

Medical Records

If patients choose to follow Dr. Coussons at Advanced Care for Women, this integration means that all your records from 2009 until today are accessible and visible in both Epic and Athena Health. The promise and advantages of a universal medical record are not yet realized, but Epic and Athena have allowed sharing. Local patient records between Women’s Specialty Care, Bellin, Aurora, HSHS St Vincent’s and St Mary’s are all connected.

Your local hospital records are available to Dr. Coussons at Advanced Care for Women and any records in Dr. Coussons office will be visible in your local hospital chart.

If you are traveling from outside of the Green Bay area, we may have access to your records online, but if not, we will provide a way to transfer your historical records and will gladly provide our notes to your physician.
