As the age of the mom increases, the risk of chromosomal problems, such as Down’s Syndrome (trisomy 21) or other major chromosomal abnormalities increase. Historically more testing was offered to this group of patients after pregnancy was achieved. But it is important to plan ahead prior to pregnancy. Most early pregnancy loss is related to these major abnormalities in chromosome numbers and is not a sign of repetitive pregnancy loss. If women have had more than one pregnancy loss and they are trying again after 35 years of age, it is important to look at the history of pregnancy loss and start the discussion about repetitive pregnancy loss.

We often start the evaluation after 2 losses, and if problems are discovered that can be treated the rate of loss drops significantly in subsequent pregnancies. Other planning such as optimal nutrition, the best prenatal vitamin and addressing any medical problems is also important prior to attempting pregnancy. A preconceptual visit at any age is the best option when planning a pregnancy, but this is especially important after 35 years of age.
